As new pharmaceutical agents are introduced, it is imperative to understand polypharmacy as an evolving set of risks unique to each patient. Polypharmacy is a complex circumstance for patients who may be overmedicating due to comorbid conditions; it is especially common and problematic in older patients. Potentially inappropriate medications, associated antibiotic use risks, and classifications of drug-drug interactions are vital factors in navigating polypharmacy. A nurse practitioner (NP) who is up to date on the latest medications and their interactions will be ready to achieve greater patient outcomes.
This session will walk NPs through treating patients at risk of polypharmacy complications. After this session, NPs will be able to identify several drugs that treat chronic conditions and may yield adverse interactions with prescriptions for acute illness. NPs will also learn to confidently assess which patients may be at risk for drug-drug interactions. Practical strategies for prescribing drugs, discontinuing medications, and monitoring patients to avoid polypharmacy will be demonstrated. NPs will be prepared to improve their acute care skills by communicating and mitigating risks when treating patients.
